New Middle School Student Enrollment Forms

Welcome to Lake Norman Charter!  We look forward to seeing you at registration night.

Listed below are the forms required for registration of new middle school students. Please complete and print the required forms. 

Checklist of paperwork for registration appointment

Attendance Acknowledgement form (required)

Authorization to Release Student Records (required)

North Carolina Health Assessment Transmittal form (if applicable)
The NC Health Assessment must be completed by a physician if your student is entering a NC public school for the first time.

Economically Disadvantaged Students data collection form (if applicable)

Authorization for Prescription Medication for students form (if applicable)
Authorization of Prescription Medication form will need to be completed by the prescribing physician if your student needs to take prescription medication while at school.
