Second Payment for the Music Department Field Trip.
Please read all the guidelines and permission below before making your payment.
With making this payment, the LNC student named below has my permission to attend the field trip.
***I understand that if my child does not follow LNC behavior guidelines on the field trip, the student may be separated from other students. Depending on the severity of the behavior, the parent may be asked to pick up the student immediately.
***By making this payment, I absolve the teacher(s), chaperones, LNC and any and all members of the governing boards of any responsibility for safety, welfare, health, and well-being of the child named in payment, beyond such matters as may be called responsible care for students in the custody of a teacher/chaperone and subject to the teacher/chaperone’s clear instructions, and assume personally and exclusively all responsibility and liability for accident, injury, etc., which may occur to the child during the time of the specific activity as set forth at the beginning of the paragraph. In case of a medical emergency, I grant permission for LNC designated representatives to seek appropriate medical care and treatment for my child.
