New Elementary School Student Enrollment Forms

The forms and information below are for families of students who have been accepted for the 2024-25 school year through the lottery held in February. If you want to apply to be added to the waitlist, please review our enrollment page.

Listed below are the forms required for registration of new elementary school students accepted in the lottery. Please print and complete all forms applicable to your child and bring them with you to your registration appointment. 

• Birth certificate (Copy)

• Doctor’s copy of immunization record (Copy)

• NC health assessment transmittal form (completed by the doctor)

• 2 utility bills: gas, water or electric (Copies)

• Driver’s license (Copy)

• Attendance acknowledgement form

• Last and current school report card (if applicable) (Copies)

• Any special program documentation (IEP, 504, AIG) (Copy)

• Economically disadvantaged students data collection form (If applicable)

• Authorization for prescription medication for students form (if applicable)

• First aid form

• Authorization to release student records form (if previously attended a NC school or registered for an early learning program with CMS)
