Second graders will be traveling on December 9th to learn more about community helpers. The price of the field trip includes covers transportation costs. 

***I understand that if my child does not follow LNC behavior guidelines on the field trip the student may be separated from other students. Depending on the severity of the behavior, the parent may be asked to pick up the student immediately.

***With submitting this form, I absolve the teacher(s), chaperones, LNC and any and all members of the governing boards of any responsibility for safety, welfare, health, and well-being of the child named in payment, beyond such matters as may be called responsible care for students in the custody of a teacher/chaperone and subject to the teacher/chaperone’s clear instructions, and assume personally and exclusively all responsibility and liability for accident, injury, etc., which may occur to the child during the time of the specific activity as set forth at the beginning of the paragraph. In case of a medical emergency, I grant permission for LNC designated representatives to seek appropriate medical care and treatment for my child.
